Appointments

Everything appointments and forms...

Thank you for scheduling an appointment with ENT for Children. We are here for you every step of the way. In order to minimize office wait times it is important that you follow the process below for scheduling and preparing for your appointment.

If you are a new patient, please follow the New Patient Steps. If you are an existing patient, please follow the Existing Patient Steps.

 

New Patient Steps

  • All new patients must call to schedule an appointment. After scheduling your appointment, you will receive an email invite with your personal access to our Patient Portal provider, MyHealthRecord.
  • Fill out the Policy Holder Information (insured person’s name and date of birth).
  • Fill out the Medical History Form.
  • Arrive at your appointment with the following in hand:
  1. Printed and signed Supplementary New Patient Forms you downloaded just above.
  2. Insurance card.
  3. Photo ID.

 

IMPORTANT NOTICE FOR NEW PATIENTS
It is a legal requirement for you to bring the 3 items as requested for us to be able to treat your child for the first time. We are highlighting this so strongly because we need you to understand the importance of bringing these documents in hand. Again, they are 1. signed original copies of the Supplementary New Patient Forms for your first visit, 2. your Insurance Card and 3. your Photo ID.

If you do NOT bring the signed original Supplementary New Patient Forms, your Insurance Card and your Photo ID you will probably need to reschedule your appointment.

 

Existing Patients

  • Schedule your appointment online through your account at MyHealthRecord or call your closest office.
  • Update information in your account with us at MyHealthRecord.
  • If any of your child’s information has changed since your last visit (ex, insurance carrier, home address, medications, etc.), please update your account in the MyHealthRecord.
  • Arrive at your appointment with the following in hand:
  1. Printed and signed forms (if you are contact and requested by our office).
  2. Insurance card.
  3. Photo ID.
